Tool balancers are designed to exert a constant force as they spool and unspool.
They’re pretty cheap surplus and are rated by tension in weight - like 5 lbs

Years ago I had one of those cheesy neck stretchers that hooks on a door and uses bottle of water as a weight. I saved the collar and hooked it to a tool balancer hooked to my headboard. When my neck hurts I slip on the collar slide my but down and have pleasant constant traction as lons as I want.
